Note: The two documents listed below may serve as a quality assurance tool when meeting compliance of medical coverage policies. A short and long form are provided for reporting client visit information. The short form is designed more for the patient who is not on a wide variety of therapeutic devices and is to the point in their disease process for documentation and outcome assessment of NPPV. The longer version is meant for complete documentation and outcome data on ALL therapeutic devices that a chronic lung disease patient may use/receive.
